Informed consent form

Patients who wish to undergo surgery are required to fill out an "informed consent form" after their examination at our clinic. You will fill this form in our clinic after the examination. You can download and read the consent form from the link here. You do not need to fill out this form until you have been examined and it is certain that you will be operated on. The form is shared here for our patients who want to read the form before coming to the examination.
